Re:      IMMEDIATE TERMINATION OF LEASE/RENTAL

AGREEMENT OF APARTMENT UNIT LOCATED AT:

Dear Occupant/Resident:

This law office represents the owner of the Landlord's Apartments.  This letter shall constitute the Landlord's notice that your Rental Agreement is immediately terminated for violations of the Rental Agreement and the Residential Landlord and Tenant Act, and specifically, the unlawful using, storing, or keeping of a controlled substance [or specify other reasons under A.R.S. '33-1368(A)] .  Furthermore, we believe your continued occupancy otherwise jeopardizes the health, safety and welfare of the landlord, the management,  and other tenants.  You are hereby notified, pursuant to A.R.S. '33-1368(A), that the above violations constitute a material and irreparable breach of the lease, and your Rental Agreement is immediately terminated. 

[Optional]  Furthermore you are notified that rent due on [date rent was due] is past due. If full payment (including any applicable late fees) is not paid within five days after your receipt of this notice, the rental agreement shall also be terminated for the reason of non-payment of rent (in addition to the above termination) and an eviction action will be filed against you to recover possession of the premises, rent, late fees, and any other amounts due under the rental agreement, including attorney's fees and costs.  

Please immediately make arrangements to move out of the apartment and return the keys to the management. 

If you fail to vacate the premises immediately, we will initiate a lawsuit against you for eviction and recovery of the premises, and we will seek damages as well as attorney's fees and costs.  You can avoid a lawsuit by vacating the premises.
