Please be notified that the landlord regards you in material breach of your Rental Agreement for the following reasons:

You are advised that if such matters are not fully remedied within ten (10) days [Note: Landlord can use five day period if violation is for health and safety] from the date of your receipt of this notice, your Rental Agreement and the right to occupy the apartment will terminate and the landlord will bring legal action against you for eviction and possession.

You should immediately contact the [Landlord/Management] about making arrangements to remedy these matters. Also, please be advised that if you commit or allow another violation to occur which is the same or similar to the violations specified above, your Rental Agreement will be terminated within ten days after the landlord provides you written notice of the second violation, and after such ten day period, the landlord will file an action for eviction and pursue any other damages or remedies.
